The Westlake Police Department is proud to introduce our Special Needs and Wandering Alert Form, designed to cater to the unique requirements of our community members with autism, dementia, or other special needs. We understand that in critical situations, it is essential for our police, fire, and EMS personnel to have access to relevant information that can make a difference.
This initiative aims to equip our emergency services with valuable insights into individuals who may face challenges in communicating during emergencies or may be prone to wandering. Whether it's a crisis situation or a missing person report, this program can be a lifeline.
Individuals with special needs, or their family members on their behalf, can complete this form to provide us with critical details such as name, date of birth, physical description, emergency contact information, known triggers, behavioral patterns, and any other pertinent information. Once registered, this information is securely stored in a dedicated database that will automatically alert our emergency personnel when responding to a call involving the registered individual or their residence.
